Borderline Personality Disorder
Borderline personality disorder (BPD) is among the most difficult personality disorders to treat, as it is characterized by intense emotional instability, difficulty managing relationships, and impulsivity. Studies have shown that individuals with BPD often respond poorly to conventional therapies such as cognitive-behavioral therapy, and are more likely to drop out of treatment. Dialectical behavior therapy (DBT) is often recommended for borderline personality disorder, as it has been demonstrated to be effective in reducing symptoms.

Narcissistic Personality Disorder
Narcissistic personality disorder (NPD) is another difficult-to-treat personality disorder. Individuals with NPD often deny or minimize their symptoms, and can be resistant to treatment as they may not see any need for it. Treatment for NPD usually involves psychodynamic therapy, which seeks to increase the individual's self-awareness and understanding of their motivations.

Antisocial Personality Disorder
Antisocial personality disorder (ASPD) is another complex personality disorder that is difficult to treat. Individuals with ASPD often lack insight into their behavior, and are often resistant to treatment. Treatment for ASPD often involves cognitive-behavioral therapy as well as family therapy to help with communication and problem-solving. In addition, medications may be recommended to help with impulsivity and aggression.
