Acute hemolytic transfusion reaction (AHTR) is a potentially life-threatening condition that occurs when incompatible red blood cells are transfused into a patient. It is one of the most serious complications of a blood transfusion. The major cause of acute hemolytic transfusion reaction is an ABO blood group incompatibility. This means that the donor and recipient have different ABO blood types. The most common cause of this incompatibility is a mistake in the identification of either the donor or recipient. Other causes include clerical errors, mislabeled samples, and contaminated blood products.
