The gold standard test for chronic pancreatitis is endoscopic retrograde cholangiopancreatography (ERCP). ERCP is a procedure that combines the use of an endoscope—a thin, flexible, lighted tube—and X-rays to diagnose and treat problems in the liver, bile ducts, and pancreas. During the procedure, a doctor inserts an endoscope through the patient's mouth, throat, and stomach into the small intestine. X-rays are then used to view the ducts and to pass a special instrument through the endoscope to take a biopsy or to inject dye into the ducts.

ERCP is considered the gold standard test for diagnosing chronic pancreatitis because it allows doctors to get an accurate view of the pancreas and its surrounding structures. It can also help identify blockages, infections, or other abnormalities that may be causing or contributing to chronic pancreatitis. Additionally, it can help identify any complications that may have developed due to the condition, such as inflammation or scarring of the pancreas or bile ducts.

In addition to diagnosing chronic pancreatitis, ERCP can also be used to treat some of the complications associated with the condition. For example, it can be used to remove gallstones that may be causing blockages or to place stents to help improve drainage of the pancreatic ducts.

While ERCP is considered the gold standard test for chronic pancreatitis, other tests may also be used to diagnose the condition. These tests include abdominal imaging (such as CT or MRI scans), blood tests, and endoscopic ultrasound.
